Consent Form (Student Vs Parent Portions)

If the Student status on the database says “NA” or “Pending Parent” under consent form,  affiliates/mentors can send this tutorial to students/Parents to help them complete the form.

Mentor Liaison Training

Every affiliate must select an individual to serve as an affiliate mentor liaison. Each year the mentor liaison is required to complete ACE’s web-based training based on this Affiliate Mentor Liaison Policy Handbook
